March 2016 – A precast concrete company was fined CA$65,000 for critical injuries suffered by a worker when concrete panels fell onto him. Crews at an industrial facility were moving two 20-ft. long concrete panels, weighing 3,750 lbs., to a storage area. As the crane operator was lowering the concrete panels onto horizontal pillars on the ground, a worker walked over to the panels to remove the sling that held them from the crane. The panels tipped over and fell on the worker, causing broken bones, lacerations, and crushing injuries. An investigation by the Ministry of Labor revealed the panels were placed on uneven and sloped pillars, violating Section 45(b) of the Industrial Establishments Regulation (Ontario Regulation 851). The employer pleaded guilty to failing to ensure proper procedures were carried out. - news.ontario.ca
