Crane Accident: Indianapolis, Ind.

September 2013—A worker who was lifting a 5,000-lb. generator with a boom truck died when he was crushed as the truck tipped over. The man worked for a company that installs mobile-phone signal towers. Witnesses said that the boom-truck’s supporting outriggers were not extended and set during the lift, and that the generator’s weight caused the boom truck to tip. They said that the operator tried to move out of the way, but was killed when the crane fell. OSHA will likely investigate. – www.indystar.com
